Transition into Post 16
At CLF Post 16, we recognise that transition from secondary school is a significant step. We are committed to ensuring that every student experiences a smooth, supportive and successful move into Post 16 education.
Our transition process is bespoke where necessary and tailored to the individual needs of each young person. This may include:
- Early transition sessions and short visits to help students become familiar with the environment, staff and expectations.
- Meetings with parents/carers or corporate parents, where appropriate, to ensure that the needs of the young person are fully understood and that all relevant information is shared.
- Close liaison with previous schools and relevant professionals to support continuity of provision.
Where needed, we work with the young person to create an individual profile. This gives students the opportunity to clearly communicate:
- What support they need
- How they would like to be supported
- Any strategies that help them to succeed
These profiles are regularly reviewed by the Inclusion Team and the young person, with contributions from relevant staff, for example safeguarding or pastoral staff, to ensure support remains appropriate and effective.
Students may also access a phased or extended transition where required, helping them to build confidence and independence at a pace that suits them.
Our aim is to ensure that every student feels safe, supported and fully prepared to thrive as they move from secondary school into CLF Post 16.
